About the Role:

Are you passionate about empowering patients and transforming kidney care education? DaVita is seeking a Kidney Smart Educator & Local Coordinator to serve patients, families, and clinical teams across the Westside Division and Greater Phoenix area.

This is a dynamic dual-role position combining clinical patient education with regional administrative coordination:

  • Kidney Smart Educator: Lead impactful chronic kidney disease (CKD) classes (in-person, virtual, and telephonic) to help patients and caregivers understand kidney function, disease progression, diet/medication management, and renal replacement options (Home Dialysis, In-Center Hemodialysis, Kidney Transplant, and Conservative Care).
  • Local Coordinator: Provide critical administrative and operational support to Kidney Smart Educators across the Westside Division—coordinating class schedules, managing physician referral intake, driving patient outreach/scheduling, and tracking post-class outcomes.

Key Responsibilities:

Patient Education & Clinical Outreach (Kidney Smart Educator):

  • Facilitate CKD Education: Deliver engaging, instructor-led Kidney Smart curriculum classes in-person across local facilities/partner clinics, as well as virtually and telephonically.
  • Empower Patients & Families: Educate patients on kidney health fundamentals, disease management, lifestyle adjustments, and treatment modalities (Peritoneal Dialysis, Home Hemodialysis, In-Center Hemodialysis, and Transplant).
  • Partner with Care Teams: Build collaborative relationships with local nephrologists, clinical practices, and dialysis care teams to expand patient access to education.

Division Coordination & Operations (Local Coordinator):

  • Administrative Leadership: Serve as the central point of contact for the Westside Division Kidney Smart team, assisting the Division Lead and field operations in expanding educational reach.
  • Class & Schedule Management: Coordinate and publish class calendars, manage room/venue logistics, and maintain educator coverage across the division.
  • Intake & Patient Scheduling: Manage intake pipelines for patient recommendations and physician referrals; execute direct patient outreach and enrollment.
  • Reporting & Compliance: Conduct post-class follow-up, track attendance metrics, submit required reporting tools, and ensure alignment with national Kidney Smart operational standards.

Qualifications & Requirements:

  • Clinical Background / Licensure: Active clinical credential/license required (e.g., Certified Clinical Hemodialysis Technician / CCHT, LPN, LVN, RN, or equivalent clinical licensure).
  • Experience: Strong background in nephrology, dialysis, or chronic disease patient education preferred.
  • Language Skills: Bilingual in English and Spanish strongly preferred.
  • Communication & Presentation: Exceptional public speaking, teaching, and interpersonal skills with the ability to connect with diverse patient populations.
  • Organizational Skills: Demonstrated strength in scheduling, workflow coordination, administrative tracking, and cross-functional team support.
  • Mobility & Travel: Reliable transportation and willingness to travel up to 20% within the assigned Phoenix metropolitan territory.
