Your Opportunity

We are seeking a Practice Transformation Manager to serve as the primary point of contact between Vytalize Health and assigned provider practices, leading the on-the-ground implementation of value-based care programs.

This role drives practice-level adoption of population health management strategies, clinical workflow improvements, and quality performance initiatives that improve patient outcomes and advance VBC contract performance.

Practice Implementation & Transformation

  • Lead the implementation and deployment of value-based care models including population health management, practice transformation, care coordination, and risk-based contracting

  • Collaborate with providers and practice staff to optimize clinical and administrative workflows using evidence-based guidelines, care pathways, and quality improvement frameworks

  • Implement and optimize practice use of technology solutions including EHRs, patient portals, and care management tools

  • Ensure compliance with all relevant regulations and accreditations related to value-based care

Population Health & Quality Performance

  • Develop and implement population health management programs to address the needs of high-risk patient populations and increase patient engagement

  • Partner with practices to identify and execute strategies to achieve quality performance goals, adapting approaches based on the needs of the practice and patient population

  • Facilitate care coordination among healthcare providers, patients, and families to ensure seamless delivery of care

  • Assess and manage risk associated with value-based care contracts, including risk stratification and mitigation strategies

Data & Performance Measurement

  • Develop and implement performance measurement systems to evaluate the effectiveness of VBC interventions and monitor practice accountability

  • Utilize all available data sources to identify improvement opportunities, measure outcomes, and inform decision-making

Relationship Management & Collaboration

  • Build strong, trust-based relationships with practice administrators, providers, and staff to drive collaboration and sustainable change

  • Collaborate regularly with Medical Directors, Regional VPs, and internal Vytalize teams to align on practice priorities and share best practices

What will make you successful here

Education

  • Bachelor's degree in Healthcare Administration, Public Health, Nursing, or a related field

Experience

  • 3+ years of experience in practice transformation, care coordination, population health, or a related healthcare operations role

  • Direct experience working with physician practices or provider organizations in a value-based care environment

Skills & Competencies

  • Strong understanding of value-based care models and primary care practice operations

  • Ability to manage multiple practice relationships simultaneously while delivering measurable results

  • Effective communicator with strong presentation, follow-up, and stakeholder management skills

Comfortable working with data and performance metrics to guide practice conversations and interventions

Travel Requirements

  • Must travel up to 25% of the time to assigned practices within the region

PREFERRED QUALIFICATIONS

  • Bilingual; Spanish

  • Clinical background or direct experience in a primary care or ambulatory care setting

  • Familiarity with EMR systems, HEDIS measures, and population health management platforms

  • Experience with ACO, MSSP, or private payer value-based care program requirements
